At Keilor Dental Group we welcome all new patients to our clinic. We offer bulk billing for Child Dental Benefits Scheme (CDBS) and a 10% discount per visit to all pensioners.* We accept insurance claims from all health insurance companies and provide bulk billings to patients under the Veteran Affairs Department Scheme.

Once you have booked an appointment with our friendly staff, you can print the following patient history form and fill it out to the best of your knowledge in order to save valuable time upon your first visit to our clinic. During your visit please bring the patient history form and give it to the dentist. If you are unable to print the form at home, you can still fill the form upon arrival to your appointment.
In order for this dental practice to provide the highest standard of care, it is requested that you fill in the patient history form carefully and thoroughly to the best of your knowledge. Any failure to make a full disclosure may place you at undue medical risk. To ensure that the details we have are accurate, please inform the dentist of any changes to your contact details and medical conditions.
